The South East Toronto FHT offers a number of services via referrals by doctors in the local area, as well as a number of programs promoting health and disease prevention.

Physiotherapist

You can benefit from the new Physiotherapy service at SETFHT if you meet any of the following criteria:

  • You are below 65 years of age and experiencing pain, mobility or with rehab needs with no or low private health insurance coverage to access physio service
  • You are below 65 years of age and currently take opioids for pain management with no or low private health insurance coverage to access physio service

Psychologist

A psychologist studies how we think, feel and behave from a scientific viewpoint and applies this knowledge to help people understand and change their behaviour.

Clinical psychologists assess and treat adjustment, behavioural and mental health difficulties across the life span through an integration of science, theory and clinical knowledge.

 

Chiropody
Chiropody involves an assessment of the foot. After the assessment, you may get advice on how to:
  • Treat and prevent disease of the foot; and
  • Treat and prevent foot disorders.

Treatment and prevention may involve therapy, orthotics that fit inside your shoes, and pain relief.
 

Nurse Practitioner

Our Nurse Practitioners are Registered Nurses who have had more university training to increase their skills in treating,diagnosing, counselling, and educating patients.  Nurse Practitioners can diagnose and treat illness. They can also take the time to make sure that you and your family members have the answers and information you need to better manage your health.

Physician Assistant

Registered Dietitian

A Registered Dietitian is an expert in nutrition and can provide you with advice, education, and plans that relate to both food and a healthy lifestyle. Dietitians have access to resources and information from Dietitians of Canada and will offer these to you in support of your goals. 

Registered Nurse

Registered Nurses ( RNs) work to promote good health and prevent illness.

They educate patients and the public about medical conditions. They provide treatments to patients and offer advice and emotional support to patients and their families. 

 

Social Work

Social Workers are professionals who help children and adults, families, groups of people, and communitites to enhance their well being. As part of a health care team, a social worker aims to help people develop skills that will help them solve problems in their own lives and in their community. Social workers deal with both personal problems and also with broader social issues such as poverty, unemployment, and domestic violence.

Social Workers can help you:

  • name and understand the sources of stress in your life;

  • find ways to cope with and solve your problems; and 

  • provide you with information or refer you to other programs and counselling services.

     

Clinical Pharmacist

The role of the Pharmacist has changed over the years. Today, pharmacists work with people of all ages to:

  • Help you use medication in a safe and effective way;

  • Prevent disease; and

  • Promote health and wellness.

Care Navigator

The Care Navigator can help you find resources that will help you and your family. With a background in social work and a strong knowledge of our community's resources, our Care Navigator will work with you to help you find all the resources you need to manage your care.
